How to Download Ayushman Card
🔹 Via Website:
Visit beneficiary.nha.gov.in.
Select “Beneficiary” and login using mobile number.
Enter your state, scheme (PM-JAY), district, etc. and search.
If the card is available, click on “Download Card”.
Verify using Aadhaar OTP and download the card in PDF format.
Online Application Process
Visit pmjay.gov.in.
Click on “Am I Eligible” and login using OTP.
If eligible, click on “Click Here to Enroll”.
Fill in the required details and upload documents.
After verification, you will receive an application reference number.
Eligibility for Ayushman Card
Your name should be listed in the 2011 SECC (Socio-Economic Caste Census) data.
How to check eligibility:
Visit pmjay.gov.in.
Click on “Am I Eligible”.
Enter your mobile number and verify using OTP.
Select your state and district, then search using your Aadhaar or other details.

Address Proof (Ration Card, Electricity Bill, etc.)
Income Certificate
Family Photograph
